The new review examined data from 17 clinical trials with a total of 20,342 participants, all focusing on the impact of anti-amyloid drugs in people with mild cognitive impairment or mild dementia due to Alzheimer’s disease. Proponents of these drugs have theorized that they would be most effective in these early stages, before the disease has progressed.

The research revealed that the absolute effects of anti-amyloid drugs on cognitive decline and dementia severity were non-existent or insignificantbeing well below the thresholds established for the minimum clinically important difference.

“Unfortunately, the evidence suggests that these drugs do not have a significant effect on patients,” emphasizes the lead author, Francesco Noninoneurologist and epidemiologist IRCCS Institute of Neurological Sciences from Bologna.

“There is now a compelling body of evidence converging on the conclusion that there is no clinically significant effect. While early trials showed statistically significant results, it is important to distinguish between this and clinical relevance. “It is common for trials to find statistically significant results that do not translate into a meaningful clinical difference for patients,” he points out.

Key result: no clinically relevant improvements

In addition to the lack of clinically significant effects, the review found that anti-amyloid drugs probably increase the risk of brain inflammation and bleeding. This was seen on brain scans with no apparent symptoms in most patients, although long-term effects remain uncertain as symptom reporting was inconsistent across trials.

Based on the evidence, the authors conclude that future trials targeting beta-amyloid peptide removal are unlikely to provide clear benefit to patients. Although these drugs manage to eliminate amyloid proteins from the brain, this does not translate into significant clinical benefit. They recommend that future research on Alzheimer’s treatment focus on other mechanisms, and that numerous studies be conducted in other directions.

“I see patients with Alzheimer’s in my office every week and I would like to be able to offer them effective treatment,” says the lead author, Edo Richard, Professor of Neurology at Radboud University Medical Center.

“Currently approved drugs offer some benefit to some patients, but there remains a great need for more effective treatments. Unfortunately, anti-amyloid drugs do not offer this solution and carry additional risks. Given the lack of correlation between amyloid clearance and clinical benefit, we must explore other avenues to address this devastating disease,” he says.
